Survey and form templates

Twenty-eight ready-made templates: feedback, leads, HR, events, research, quizzes and polls. A template opens as an ordinary survey with the questions, the order and the settings already in place, and every line of it is yours to edit.

The whole catalog

Names and descriptions match the builder, so the card you pick here is the card you will recognise inside the app.

Feedback

Reviews, support ratings and loyalty measurements. This is where NPS, CSAT and the plain «tell us what went wrong» form live.

Leads and contacts

Requests, briefs and bookings. The questions are written so the sales call starts from the budget, not from scratch.

HR and team

Candidate screening, onboarding, the weekly pulse and eNPS. Everything a company asks its own people rather than the market.

Events

Registration, post-event feedback and age-gated sign-up. Handed out as a chat link or as a QR code on a badge.

Research

Matrices, ranking and pairwise comparison: the question types that pull out priority instead of opinion.

Quizzes

Tests with right answers, scoring and different endings. Plus a lead-gen quiz that finishes on a contact form.

Voting

One vote per device ships switched on inside the template: without that setting a poll counts wrong and you find out from the ballot stuffing.

Showcase

A demonstration of how far the styling goes: a custom theme, a dark canvas and one question per screen.

How to build a survey from a template

Four steps from a card in the catalog to a link you can already send out.

  1. Pick a template

Open the catalog and filter by category: feedback, leads, HR, events, research, quizzes, voting. Take the closest one, an exact match is not the point.

  2. Rewrite the questions

The template opens as an ordinary survey: text is edited in place, spare blocks are deleted, new ones come from a palette of 43 question types.

  3. Check the paths

The canvas shows where every answer leads. The editor highlights unreachable questions and loops before you publish, not after the first responses land.

  4. Publish and share the link

A survey ships as a link, a QR code or a one-line website widget. Responses land in your account and on a webhook, in Telegram or in a CRM.

Frequently asked questions

Survey templates: common questions

What is a survey template?

A finished survey with the questions written, the order set and the settings applied. You open it in the builder and adapt it, instead of facing an empty canvas and inventing wording from nothing.

Are the templates free?

Yes, the catalog is open on the free plan and no card is required. The free plan limits volume, not templates: 5 surveys and 150 responses in total. Half of the templates are built from free question types only; in the rest the paid block carries a badge.

Can a template be changed?

Completely. A template is a starting point, not a locked form: any text can be rewritten, any question removed, the paths between questions and the styling changed. The original stays in the catalog, you get your own copy.

What if the template I need is missing?

Describe the job in plain words and AI generation will draft the survey; three generations a month are included in the free plan. Or take a neighbouring template from the same category and rewrite the questions, which is usually faster than starting empty.

How is a template different from starting from scratch?

It saves you the wording and the structure. Required fields are already marked, consent sits where the law expects it, and the order is sane: contacts come after the person is engaged, not on the first screen.

Can a survey from a template be embedded on a site?

Yes. Every survey publishes three ways: as its own page behind a link, as a QR code and as a website widget in one script tag. The widget runs inside a Shadow DOM, so your site styles cannot break it.

Do not start from an empty canvas

Open the catalog, take the closest template and rewrite the questions for your job. Five minutes instead of an hour.
