Field validation
Email, phone, date and number fields check their format. Required fields stop empty submissions. Custom regex rules are on the Pro plan.
Contact forms, request forms, registrations, questionnaires and surveys. Add fields, set validation and conditions, then embed the form with one line of code or share a link. Free plan, no card.
Open the builder, add fields (name, phone, email, a dropdown, a comment box) and mark the required ones. Email and phone formats are validated automatically. Publish the form and pick how to show it: a link, a QR code or a widget on your site. Responses collect in your inbox, and from the Pro plan they also go to your webhook.
Below are the form types people build most often in Askyo, each with its own page, plus what to know before you start: where submissions go, what the free plan covers and what the service does not do.
One engine, different jobs. Each type has its own page with example fields and settings.
Name, phone, what they need. Format validation, required fields, a captcha against spam, and delivery to your webhook.
Feedback formLonger data collection with input validation, conditional sections and a spreadsheet export.
Questionnaire builderSign-ups for a webinar, conference or workshop, with date and format choices and a consent checkbox.
Registration formA short on-page survey, as a popup or inline block. Works on WordPress, Webflow and any HTML site.
Website surveyA project brief for a website, design or campaign: a multi-page form with file uploads and branches per project type. Start from a template.
Client brief templateOne 0–10 scale and a “why” question. The Net Promoter Score is calculated for you.
NPS surveyAnswer keys, points and an “X out of Y” result for classes and training.
Test makerA few questions about the visitor’s needs and a contact form at the end, with different endings for different answers.
Quiz makerPick an option with one vote per device. Only the organizer sees the results.
Online votingTen ready-made questionnaires for schools and preschools with every question written out.
Parent surveysThere are three ways to show a form. A link to a hosted page works for email and chat. A QR code works for print and events. For your site there is a widget: one <script> line where the form should appear.
The widget renders inside a Shadow DOM and weighs about 35 KB, so your site’s CSS cannot break the form and the form cannot break your site. Paste it into an HTML block in WordPress, Webflow, Squarespace or your own code. Developers get a JS API and lifecycle events: see the widget page, and the website survey page for ready scenarios.
Every extra field on a contact form costs you submissions. Ask only for what you need to reply, and get the rest in the conversation.
Email, phone, date and number fields check their format. Required fields stop empty submissions. Custom regex rules are on the Pro plan.
Show a field only to the people who need it, such as “Company name” after “I am a business”. Conditions live on the logic graph; the free plan allows up to three branches from one question.
Résumés, briefs, photos. File upload is on the Pro plan with files up to 10 MB, and larger on higher plans.
A captcha block stops the bots that fill in website forms. Available on the Pro plan.
All fields in a list, one question per screen, page by page, or as a chat. Split a long questionnaire into pages and keep a short contact form as a list.
A consent block with a policy link is on every plan. A drawn signature field, by finger or mouse, is on the Team plan.
Every response lands in your Askyo inbox with a per-question summary. After that it depends on the plan.
From an empty form to submissions arriving at your webhook. Signing up is free.
Open app.askyo.ru/build and start from scratch or a template. You can also describe the form to Yo, the AI assistant, and get a draft.
Add name, email, phone and your questions. Mark the required fields; phone and email validation switches on automatically.
If some fields only apply to some people, show them conditionally on the logic graph. Add a consent block linked to your privacy policy if you need one.
Publish the form and copy the widget code. Paste it into an HTML block on your page, or share the link and QR code.
On the Pro plan, add a webhook or connect Make or Zapier so submissions reach your tools. Send a test submission and check it arrived.
Yes. The free plan includes 5 forms and up to 150 responses, with field validation, conditional logic and the website widget. Webhooks, captcha, file uploads and CSV export start on the Pro plan.
Publish the form and copy the widget code: a single <script> line. Paste it into an HTML block in WordPress, Webflow or your own template. The widget is isolated in a Shadow DOM, so your site’s styles do not leak into it.
No, Askyo does not send email. Submissions collect in your inbox, and from the Pro plan they can go to a webhook, Make, Zapier or Telegram, which can then notify you however you like.
In Askyo they are one tool. A form collects details about one person: a request, a registration, an application. A survey collects opinions from many people for a summary. The fields, validation and conditions are the same; only the job differs.
Yes. Besides the cloud version there is a self-hosted build for teams that need form data to stay on infrastructure they control. It is licensed separately; details are on the pricing page.
Free, no card needed. The widget code is ready in five minutes.
